Yes. Our ARF + FCA Fast-Track service pairs you with a registered OT who completes your Functional Capacity Assessment and Access Request Form with you, face-to-face or via telehealth, for a capped $1,499. You’ll get a clear checklist, evidence summary and guidance on what happens next.
We’re based at 9 Kraft Court, Broadmeadows VIC 3047 and support participants across Melbourne’s North & North-West. Most services are delivered in-home, in the community or at our centres, with telehealth available for suitable supports (e.g., assessments, reviews, coordination).
Yes. Our service bundles include Daily Living, Life Skills & Transitions, Community & Participation, Transport, Employment Support, Support Coordination and High-Intensity & Clinical. We’re registered for Complex Bowel Care and Subcutaneous Injections, with Community Nursing being added.
Day Programmes run in small groups with a goal-based timetable (e.g., cooking, fitness, art, local outings). Funding usually comes under NDIS Group/Centre-Based Activities (0136). Transport is available within our service area. You’re welcome to book a trial day to see if it’s the right fit.
